Please complete this form to apply for withdrawl of your units.

The terms and conditions for withdrawing your investment in Fund (Fund) will be stated in the relevant Information Memorandum (IM), together with the other important information taken to form part of the IM. From time to time, Specialised Investment and Lending Coporation Pty Ltd (the Trustee of the Fund) may update these documents. Please enter either the Australian dollar amount or number of units you wish to withdraw below. This request must meet the terms andconditions for withdrawing your investment as set out in the IM, otherwise the Trustee reserves the right to reject your withdrawal requestor treat the instruction as a full redemption.

The bank account details you provide above will replace the previously nominated account and will be held on record and maintained to payany future withdrawal proceeds and/or income distributions. This account must be with an Australian Authorised Deposit-taking Institution (ADI) and must be in the name of the investor as we will not pay to a third party or offshore bank account. Please check these details carefully as it is your responsibility to ensure all payee account details are correct. Incorrect details may result in a loss of funds and we donot guarantee their recovery. We do not accept liability for funds which are unable to be recovered.

D. Declaration and Signature

Please read the declarations below before signing this Withdrawal Request Form.

By signing the below, I/we:

  • declare that I/we have read and understand the current (and any Supplementary) IM for the Fund;

  • declare that all details provided in this Withdrawal Request form are true and correct and I/we undertake to inform you of any changes to the information supplied as and when they occur;

  • (If signing under power of attorney) declare that I/we have not received notice of revocation of that power;

  • authorise Trustee to act upon instructions by post, facsimile or email (as applicable) with regard to the units in Fund subscribed for (and any further units acquired) or any matter in connection with them or any of them without liability in respect of any transfer, payment or any other act done in accordance with such instructions and notwithstanding the same was not signed or sent by me/us. I/we agree that this authorisation shall remain in force until notice in writing of its termination is received by Trustee.

Who must sign:

All investors, trustees, partners or office bearers must sign. If a company, two directors, a director and a company secretary, a sole director and sole company secretary or duly appointed attorneys must sign. If signing under a power of attorney, they must have previously been noted by the registry.
